The microenvironment of solid tumors is mostly characterized by regions of acute and chronic hypoxia which are known to decrease sensitivity of tumor cell to classical chemotherapy and radiotherapy. Here is proposed a novel strategy to overcome therapy resistance of solid tumor cells under acute hypoxia and to increase the efficacy of radiotherapy by using the radical-forming endoperoxide Dihydroartemisinin (DHA) that is known as anti-malaria drug and has only recently been shown to have antineoplastic activity. Aim of the current study was to evaluate the antineoplastic activity of DHA under normoxic and hypoxic conditions, to elucidate the importance of the intrinsic apoptotic pathway for drug activity in both conditions, and to study its potential to improve the cytotoxic action of ionizing radiation. The cytotoxic effects of DHA were evaluated in colon cancer cells (HCT15, Colo205 and HCT116) and Jurkat T-lymphoma cells using short term and long term assays. For combined treatment, ionizing radiation was applied 10 minutes after DHA treatment. In normoxia, DHA efficiently induced concentration- and time-dependent apoptosis in Jurkat and colon cancer cells. The cells were also sensitive to DHA-induced apoptosis in hypoxia. Combination treatment with XRT increased clonogenic death in HCT116 cells. All together findings of the current work suggest that DHA may be a good candidate for the treatment of hypoxic human tumors characterized by apoptosis resistance and for combined treatment with ionizing radiation.

Das Mikromilieu solider Tumore ist häufig durch Areale akuter und chronischer Hypoxie gekennzeichnet, welche die Sensitivität von Tumorzellen gegenüber klassischer Chemo- und Radiotherapie verringern. In der vorliegenden Arbeit sollte eine neuartige Strategie untersucht werden, um einer Therapieresistenz solider Tumorzellen bei akuter Hypoxie entgegen zu wirken. Aus früheren Arbeiten ist bekannt, dass das in der Malaria-Prophylaxe verwendete radikalbildende Endoperoxid Dihydroartemisinin (DHA) antineoplastische Aktivität zeigt. Im Rahmen dieser Arbeit wurde nun untersucht, welchen antineoplastischen Effekt DHA in Normoxie bzw. Hypoxie zeigt, welche Bedeutung dem intrinsischen Apoptoseweg für die zytotoxische Wirksamkeit von DHA zu kommt und welches Potential DHA besitzt, um die Zytotoxizität durch ionisierende Strahlung (XRT) zu verbessern. Um dies zu untersuchen, wurde die zytotoxische Wirksamkeit von DHA in Kurzzeit-Assays und Langzeit-Assays in Kolonkrebszelllinien (HCT15, Colo205 und HCT116) und Jurkat T-Lymphomzellen betrachtet. Bei einer kombinierten Behandlung von DHA und XRT erfolgte die Bestrahlung 10 min nach DHA Gabe. Unter normoxischen Bedingungen induzierte die Behandlung mit DHA eine konzentrations- und zeitabhängige Apoptose in Jurkat- und Kolonkrebszellen. Des Weiteren waren die Zellen unter hypoxischen Bedingungen sensitiv gegenüber der DHA-induzierten Apoptose. Eine kombinierte Behandlung von HCT116 mit XRT und DHA führte in Normoxie zu einem verstärkten klonogenen Tod. Insgesamt zeigt die Arbeit, dass DHA ein potentieller Kandidat für die Behandlung hypoxischer Tumore alleine und in Kombination mit Bestrahlung ist.
